Book an Appointment
Step 1. Verify Benefits:
*BCBS, Cigna and Anthem PPO beneficiaries can verify HERE.
*United Healthcare PPO beneficiaries can verify here.
*All Tricare beneficiaries are eligible under CBSD with no referral needed.
*If you do not qualify for coverage under your insurance, please review the Services Menu for self-pay options. I do offer discounted pricing for Medicaid beneficiaries as I am unable to bill to Medicaid at this time.
Step 2. Book here for in-office visits. If you have verified your commercial insurance benefits or if you are a Tricare beneficiary, please ensure you select the verified insurance benefits option. A credit card will still be collected at booking but you will not be charged for the appointment.
If you would to schedule a home visit using the self-pay option, please send me a message via the contact form, Facebook or my phone number listed at the bottom of the page. Please ensure you input your email address and phone number correctly so that I can reach out to you if needed. Within 24 hours of booking, you will receive emails containing links to 3 intake forms as well as an invitation to the secure messaging portal.
Step 3. If you are an existing client trying to access your portal, you can do so here. From your portal you can book future appointments, update your contact information, upload documents and send secure messages.
**CANCELLATION POLICY: In an effort to serve as many mothers as possible, cancellations need to be made as soon as you are aware of any issues. Although we try to be as flexible as possible, any appointments cancelled less than 24 hours prior to the appointment time, as well as any no shows, will be charged a $75 appointment fee with the card on file. By booking this appointment, you express agreement with this policy.**
4. On the day of your appointment, please ensure baby is ready to nurse upon arrival or within 30 minutes of arrival. If you are not sure how to time this, please feel free to reach out for suggestions. Bring any supplies relevant to our appointment with (or have them prepared if I am coming to your home). If you are having serious supply issues, please bring one feeding’s worth of a supplement (breastmilk or formula) with you.
****If you have reached out to me due to having difficulty booking or booking a home visit, please ensure you promptly pay your invoice once I have confirmed your appointment with you (if you are self-pay). If your invoice is not paid within 48 hours of booking or no less than 48 hours prior to your appointment time(whichever comes first), your appointment will be cancelled in order for me to allow time to book other clients. I will not send reminders for this as I have a busy practice and many clients that I serve and maintain contact with on a daily basis.****
