Book an Appointment

Step 1. Verify Benefits:
*VA CCN, Aetna, Anthem, BCBS and United Health Care PPO beneficiaries can verify here. (Please note that depending on how you mark the urgency of your requests, responses could take 1-5 days)
*CIGNA beneficiaries can verify here. (Responses within 24 hours)
*No longer accepting NEW Tricare clients without a faxed referral AND extenuating circumstances; or unless you are a returning client or a client of Dove Pediatrics.
*If you do not qualify for coverage under your insurance or are opting for self-pay, please review the Service Menu for pricing. Medicaid pricing is 50% off of the listed amount.

Step 2. Send a Contact Form
*Once you have verified your commercial insurance, have been accepted as a Tricare client or have opted for self-pay, please send a contact form with how soon you would like to be seen and any non-availability dates/times. Please ensure your contact information is accurate so as not to delay me reaching out to you.  (If for any reason my contact form is not working, please feel free to reach out via the Facebook business page.)

Step 3. Return Completed Intake Forms
*Once I have reached out and we have agreed on a date and time for your appointment, you will be sent intake forms via e-mail(check Spam if you do not see them). They will need to be filled out within 24 hours in order to secure your appointment slot.

**CANCELLATION POLICY: In an effort to serve as many mothers as possible, cancellations need to be made as soon as you are aware of any issues. Although we try to be as flexible as possible, any appointments cancelled less than 24 hours prior to the appointment time, as well as any no shows, will be charged a $75 appointment fee with the card on file. By booking this appointment, you express agreement with this policy.**


Step 4. Prepare for you appointment
*On the day of your appointment, please ensure baby is ready to nurse within 30 minutes of arrival (if you are planning to nurse in office or would like to do a weighted feed).
*Bring any supplies relevant to our appointment with (or have them prepared if I am coming to your home). If you are having  supply issues, please bring one feeding’s worth of a supplement (breastmilk/formula) with you. 
*Upon arrival, have a seat. I do not have an office assistant and I sometimes run behind but will be with you shortly!
